UTAH ID CARD

IF YOU HAVE HAD A UTAH ID IN THE PAST FIVE YEARS:

You will need:

  1. ID Voucher (from VOA, Library Social Worker, or Crossroads Urban Center)

IF YOU HAVE NEVER HAD A UTAH ID:

You will need:

  1. Hard Copy of birth certificate and Social Security card
  2. ID Voucher (see above)

Take these documents to one of the Drivers License Division offices

  • Fairgrounds: 1095 Motor Ave SLC, UT 84116
  • West Valley: 2780 West 4700 South Taylorsville, UT 84129

*If you can fill out the application online beforehand, this can save time.

Once you get a temporary copy of the ID from the DLD, the hard copy will be mailed to you.


GETTING YOUR SOCIAL SECURITY CARD

Start here if you currently don’t have any documents

You will need:

Original Medical Demographic form from 4th Street Clinic or Sacred Circle. They will also accept immigration documents.

You will need to fill out and sign a Social Security card application and take the form and documents directly to or mail to:

If you currently have valid photo ID

Bring your ID to the above Social Security Admin address and apply for a replacement card in person. Temporary IDs are fine, but copies of IDs are not.


GETTING YOUR BIRTH CERTIFICATE

If you were born in Utah, you will need:

  1. Copy of ID or see list of other documents on the the Birth Certificate application
  2. Birth Certificate Voucher (from VOA, Library Social Worker, or Crossroads Urban Center)
  3. Birth Certificate Application

You will need to fill out Birth Certificate application form and take form and documents to:

If you were NOT born in Utah:
  1. You will need to know the specific State and County you were born and print the Birth Certificate application from the official state/government website
  2. Some states may offer vouchers that pay for Birth Certificates for homeless individuals. CA and NM both do. Print out this form if available. 
  3. Financial assistance may be available from the Weigand Center 801-363-7710 or Jewish Family Services 801-746-4334.
  4. Mail signed application and required documents to the state’s Vital Statistics office.
