The Salt Lake City Public Library is an official passport application acceptance facility. Customers can apply for a U.S. passport and/or passport card.
You must apply in person if any of the following are true:
Applicants will be seen during office hours on a first-come, first-served basis. Please be aware that each acceptance/execution will require 20 minutes, assuming the application is filled out completely and correctly and all documents and forms of payments are present.
Passport Services are offered at the Main Library, Level 3.

Proof of Identity
Approved forms of ID:
• Utah Driver’s License
• Utah State ID card
• Previous Passport
• Certificate of Naturalization with identifiable photo
• Certificate of Citizenship with identifiable photo
• Government Employee Identification with identifiable photo
• Official U.S. military or military dependent ID
• Current (valid) foreign passport
Proof of U.S. Citizenship
Submit an original AND a photocopy.
Documents are submitted with the application and will be mailed back to you by the State Department.
Acceptable proof of citizenship includes:
• Naturalization Certificate
• Previous U.S. Passport (may be expired but must be undamaged)
• U.S. Birth Certificate *
• Must have been taken in the past six months
• Front view with a white background
• Must be 2" x 2" in size
• Library can take a passport photo for you ($15 Fee)
NOTE: Passport applications for minors under age 16 may require additional documentation. See the U.S. Department of State website for more information.
* Must be an original or certified copy and list your full name, date and place of birth, and parent(s) full names. If you need a copy of your Utah birth certificate, contact the Utah Vital Records Office.

** Requires that you appear in-person at a Passport Agency or Center. The City Library is NOT a Passport Agency or Center, but rather a Passport Acceptance Facility. Based on need; restrictions apply.
NOTE: Processing times only include the time your application is at one of the Passport Agencies or Centers. Mailing times are not included in processing times. The total time to get your passport includes both processing and mailing times.
It may take up to 2 weeks for applications to arrive by mail at a Passport Agency or Center, and up to 2 weeks for you to receive a completed passport by mail after it is printed.
Consider the total time it will take to receive your passport when you are booking travel.
